• 1 Styrofoam container
• 5 green top (sodium heparin) tubes
• 1 lavender top (EDTA) tubes
• 2 gray/red top (serum separator) tubes
• 1 absorbent pad
• 1 silver thermo-pack
• 1 FedEx air bill
• 1 FedEx Clinical Pack shipping envelope
• 1 VIP Dx test requisition form
• 1 Shipping instruction sheet
• 1 Collection instruction sheet

• Place specimen tubes into Styrofoam container
• Place absorbent pad over specimen tubes
• Insert Styrofoam container into silver thermo-pack
• Insert thermo-pack into FedEx Clinical envelope
• Place (adhere) FedEx preprinted airbill on FedEx Clinical envelope
• Call FedEx for "priority overnight" pickup
• VIP Dx pays for all shipping charges to/from patient and/or physician clinic

• Collect the required number of tubes for each test as listed under "Specimen" next to each test on the test requisition
  Example: 3GR = three green top tubes
• Clearly and properly LABEL ALL SPECIMENS to correspond with patient test requisition
• SHIP all specimens AT AMBIENT (ROOM) TEMPERATURE
• DO NOT REFRIGERATE OR FREEZE THE SPECIMENS
• The PHYSICIAN ordering the tests MUST SIGN the REQUISITION
• Ship samples as soon as possible after collection
• Samples must be received within 24 hours after draw
  • DO NOT SHIP ON FRIDAY OR THE DAY BEFORE A HOLIDAY
• Use the provided IATA approved shipping containers and FedEx clinical envelope

Common Causes for Specimen Rejection:

• Received without collection date and time noted on test requisition
• Refrigerated or frozen specimens
• Specimens received on ice or ice pack
• Specimens received in excess of 24 hours post draw
• Specimens collected in incorrect tubes for analysis
• Specimens with inadequate identification

NOTE: Tubes may be used for more than one test. For example: 1 SS and 1 L are good for a Cytokine Profile and an Immunobilan test. There are enough tubes in this kit to fit any combination of testing. If any questions, please call Lab at 775-351-1890.

• VIP Dx is closed on Saturday, Sunday and legal holidays
• Do Not ship specimens on Friday/Saturday/Sunday or before a legal holiday
• Specimens must be received within 24 hours of draw or they will be rejected
